Antenatal awareness and obstetric outcomes in large fetuses: A retrospective evaluation

Abstract Introduction : There is currently no consensus on the management of large fetuses in order to minimize fetal complications. The aim this study was assess whether antenatal recognition large-for-gestational age (LGA) reduced poor obstetric newborn outcomes a hospital where expectant used. Material and methods A retrospective cohort made two delivery units at Karolinska University Hospital, Stockholm, Sweden, using LGA. All deliveries > 37+0 weeks gestation during an 8-year period (2002–2009) were included. main outcome severe adverse outcome, composite variable including neonatal trauma (brachial plexus birth palsy [BPBP] fractures) asphyxic sequelae (severe asphyxia, cerebral damage, fetal/infant death). Results population consisted 63,542 appropriate-for-gestational (AGA) 3,343 LGA pregnancies (of which 21 % identified before delivery). Compared AGA, showed five-fold increased risk (OR 5.1, 95 CI 4.0 ˗ 6.4), but differences seen regarding sequelae. antenatally had 3.7 all cases, compared 3.5 not 1.07 0.7 ˗ 1.7). Conclusion Antenatal awareness did lower unit management, postdate trauma. For every 14 5000 g cesarean section, one case could be avoided.
